You may print and sign this agreement form and bring it with you, or sign it upon arrival before your appointment.

This is to ensure you are getting connected to the proper resources and to determine eligibility. Financial assistance is available first come, first served. Program guidelines are subject to change. An appointment does not guarantee financial assistance. A maximum dollar amount has been established, is subject to change, and will not be exceeded per request. Financial assistance is based on funding availability and your household’s unique circumstances. That is why LCCM reviews requests for help on a case-by-case basis
- Bank statement (this is for you to reference to during your appointment as we fill out your monthly income and expenses)
- Photo ID
- Utility bill you are seeking assistance for. Bring the regular monthly bill as well as the shut-off notice if you have one. Bring any other utility bills you may be struggling with so we can provide guidance.
- Names and dates of birth for other household members
- Information about household income for the past 30 days (i.e. paycheck, direct deposit statements, SSI letter, etc.). Please provide any further documentation that clarifies your hardship:
- Separation letter for a job loss
- Paycheck stubs that indicate what you were paid before and hours worked, as well as paystubs that show the decrease in hours and pay
- Emergency expenses, receipts that indicate PAID for expenses (house repairs, car repairs, medical bills)
